• Print the Student Registration / Release Form  (One for each participating student in your family)                     
• Complete ALL Required Fields
• Bring the completed form to class
• Submit the form to Mrs. Makmur, our Dojo Administrator
• Mrs. "M" will introduce you as new students to the instructors

• A $20 Test fee per student is requested if a member is invited to take a promotion test.
• Uniform/patch must be purchased by students who would like to be recognized as a 
  member of the program. 
• Only students with a full uniform with a Piscataway Martial Arts patch are eligible to participate in promotion testing.
• Uniform sets are available for $45.00.  Sets include Jacket, Pants, White Belt, Embroidered Patch, T-Shirt with Logo
• See Mrs. Makmur to order a proper fitting uniform for your child.  Uniforms sets must be paid-for in advance.

• Summer 2009 Training is held at Piscataway High School at the Back Gym
• Held on Tuesdays and Thursdays beginning July 7, 2009;  7:00 to 8:00 ONLY for the Children's Class
• Last class for Summer 2009 at the High School is on Thursday, August 27, 2009
• The 2009-2010 Training is held at Conackamack Middle School
• Held on Tuesdays and Fridays beginning September 15, 2009;  Regular training hours as posted below

• Dojo Doors are opened at 6:45 pm to allow for student preparation, personal warm-up, dojo administration
• Class Starts Promptly at 7:00 pm

• September to June Session is held at Conackamack Middle School, 5202 Witherspoon Street, Piscataway.
• White Belt (Up to 2 Yellow Stripes)  : 7:00 pm - 8:00 pm
• Yellow Belt and Higher                     : 7:00 pm - 8:45 pm (with water break)
• July and August Session is held at Piscataway High School  (Back Building Gym)
• White Belt (Up to 2 Yellow Stripes)  : 7:00 pm - 7:30 pm
• Yellow Belt and Higher                     : 7:00 pm - 8:00 pm (with water break)
