To Register Your Child for Martial Arts Class :

1). Complete and Submit the following 4 forms to the Dojo Administrator at the Dojo, when you attend your 1st class

o Piscataway Rec. Registration

o Piscataway Rec. Code of Conduct

o Child Registration Form

o Cuong Nhu Release Form

2). Student's Daily Checklist

o My Daily Checklist Personal Development Checklist which each student submits at the end of each month

to the instructors. Personal growth and development is an integral part of this

martial arts program.

3). For Your Records Only

o Conditions for Participation Read and Understand; Print for your own records

o NJ Model Code of Conduct Read and Understand; Print for your own records

For Ordering a Martial Arts Uniform Please have payment in CASH Only

o Uniform Order Form Print, Provide all requested information, $45.00 Cash, Submit to Mrs. M at class

For Ordering Sparring Equipment Please have payment in CASH Only

o Equipment Order Form Sparring equipment recommended for ALL Students ESPECIALLY Yellow Belts and above

For Ordering Additional Shirts Please have payment in CASH Only

o Shirt Order Form For additional T-Shirts and Sweatshirts

For the Student's Daily Checklist My Daily Checklist; Print out and check off accomplishments each day
